2. Please enter initials/surname as last name. Middle Name is not mandatory.
3. Details entered at the time of registration can’t be edited/corrected after generation of the application number.
4. Details entered in the application form can’t be edited after submitting the application form online.
5. Do not dispatch the application form or supporting documents to the Institute.
